The primary purpose of the RN Resident Assessment Coordinator (RNAC) position is to assess and/or coordinate assessment of all residents and to complete the MDS forms in accordance with State and Federal regulations and facility policies. This position is also responsible for triggers, protocols and care plans associated with the MDS per each resident.
What will I do in the role of RN Resident Assessment Coordinator (RNAC)?
- Perform assessment of all residents to generate the MDS.
- Review, sign and date all MDS forms within proper time frame.
- Implement each residents care plan through action and education of all staff involved.
- Complete data entry of MDS information into the computer and transmits data to HCFA intermediary according to state and federal requirements.
- Responsible for the accuracy of MDS assessment. Completes portions of and coordinates with pertinent team members with the timely completion of MDS assessments, CAAS and care plans.
- Assists in the collaborative assessment and care planning process.
- Participate in care plan meetings and provide input on resident care needs.
- Maintain accurate and up-to-date resident records.
- Communicate effectively with residents, families, and healthcare providers.
- Maintain good communication and share information with finance and therapy departments as it pertains to Medicare, HMO reimbursement issues.
- Responsible for the managed Medicare products, appeals and submission of weekly documentation.
What do I need in the role of RN Resident Assessment Coordinator (RNAC)?
- Must maintain current RN licensure by the Pennsylvania State Board of Nurse Examiners.
- Two (2) years of RNAC or related experience preferred.
- RAC-CT certification required.
- Strong knowledge of MDS and RAI processes.
- Effective communication and interpersonal skills.
- Proficient is relevant computer applications, knowledge of Point Click Care is a plus.
