Job Title : Case Manager

Job Location : Hybrid : remote field visits in Las Cruces, Dona Ana County, NM

Job Duration : 3 months

Shift : Monday to Friday 8 : 00 am - 5 : 00 pm

Job Description :

  • Responsible for conducting medical management and health education programs for customers on government health care programs.
  • Responsible for representing the company to members.
  • Perform assessments, care planning and care coordination in the member / family home and telephonically.
  • This role will include gathering, analyzing, and providing date for regulatory reports.
