Job Description:
Responsibilities:
- Provide clinical judgment, pharmaceutical expertise, and distribution skills to ensure optimal patient care.
- Review drug orders, patient profiles, and medical records to assess the appropriateness of drug therapy.
- Compound, prepare, and dispense medications in compliance with departmental procedures and regulations.
- Oversee shift workflow and supervise pharmacy technicians and interns.
- Serve as a resource for drug information, consulting with medical staff, nursing staff, and patients.
- Maintain accurate and complete patient profiles in the Electronic Health Record (EHR).
- Conduct pharmacy profile-nursing medication administration record (MAR) comparisons.
- Document clinical interventions, formulary compliance, and non-compliance issues.
- Identify and report adverse drug reactions (ADRs) and medication errors per department protocols.
- Initiate or adjust drug dosages, intervals, administration routes, or lab tests under approved protocols.
- Assist in quality improvement projects and data collection as assigned.
Skills & Qualifications:
- Experience: Minimum one year of hospital-based experience in unit-dose distribution and IV additives or completion of a hospital residency required.
- Pharmaceutical Knowledge: Strong understanding of drug information, therapeutics, and pharmaceutical care for geriatrics, adults, pediatrics, and neonates.
- Communication & Collaboration: Excellent verbal and written communication skills; ability to consult and collaborate with healthcare teams.
- Interpersonal Skills: Demonstrates values of Caring, Communication, Collaboration, and Competence in patient interactions.
- Physical Requirements: Ability to stand for up to 80% of a shift and lift up to 25 pounds.
- Regulatory Compliance: Knowledge of hospital pharmacy regulations, protocols, and quality assurance measures.
Education:
- Doctor of Pharmacy (Pharm D.) or Bachelor of Science Degree in Pharmacy, and evidence of required continuing education.
Licensure:
- California Board of Pharmacy licensure.
