The Intake Lead generates new patient intakes and ensures potential patients complete their initial intake appointments with a clinician. Evaluates the mental healthcare needs of clients and determines eligibility for services. Ability to effectively present information in one-on-one and small group situations to patients and their families, facility leadership, employees clients, providers, vendors, etc. 6. Ability to listen, empathize and offer constructive guidance. Essential Job Duties/Responsibilities % Of Time 1. Responds to intake calls and completes admission paperwork. Assessments will be completed by RN, LPC or LMSW. 20% 2. Conducts interviews individually or in families to assess their situation and determine types of services required. 15% 3. Tracks all inquiries and resolves issues as it pertains to the intake process. 10% 4. Collects data to help with the referral process. Follows up on referrals by contacting referral sources to collect data. 10% 5. Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities. 20% 6. Reviews newly admitted patient accounts for eligibility and authorization daily. 10% 7. Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships. 5% 8. Participates as an active member of the interdisciplinary team. 5% 9. Ensures that all Intake forms are properly completed and filed. 5% 10. Performs other duties as assigned/required by this position. 5%
