Responsibilities:
- Deliver comprehensive patient care, tailoring interventions to the complexity of medical conditions
- Process medication orders with precision, conduct thorough patient profile reviews for potential interactions and duplications, and verify medication accuracy for all age groups.
- Collaborate with physicians to resolve prescribing discrepancies and optimize drug regimens
- Implement clinical interventions within the pharmacy profile and analyze laboratory and microbiology data to assess age-specific medication responses.
- Compound or direct the preparation of sterile IV admixtures, epidural pain medications, TPN, and chemotherapy, adhering to strict aseptic techniques and safety protocols within laminar airflow hoods.
- Document patient care activities and interventions in compliance with departmental policies and procedures.
- Detect, report, and document adverse drug reactions, interactions, and allergies, and initiate appropriate interventions according to hospital policy.
- Advise physicians and nursing staff on medication availability, dosage forms, side effects, and incompatibilities. Recommend formulary alternatives to optimize therapeutic outcomes.
- Manage and facilitate pharmacy student rotations, providing valuable educational experiences.
- Provide expert drug information and literature to staff, and develop and deliver comprehensive educational programs and training sessions.
- Contribute actively to hospital committees as directed by the Pharmacy Director, representing the pharmacy department.
- Lead the development, implementation, and maintenance of clinical drug programs, and generate detailed performance reports.
Qualifications:
- Bachelor’s Degree in Pharmacy or Doctor of Pharmacy
- Current Florida Pharmacist license
- 1 year of Pharmacy experience
Compensation: Experience and education may be considered along with internal equity when job offers are extended.
Locations: Brooks Rehabilitation Inpatient Hospitals located at 3599 University Blvd South, Jacksonville, FL 32216 and 6400 Brooks Bartram Dr. Jacksonville, FL 32258
Hours: Monday - Friday, 8am - 5pm
