The Carefirst Hospice team is growing and seeking a compassionate nurse that has a desire to serve the people we love in Southern Indiana!
We are looking for a PRN Hospice Nurse (RN) to provide skilled care to clients and their families currently experiencing a complex season of life. All of our hospice team members provide a critical role in end-of-life preparation and care, including comforting interactions with the clients support system.
The ideal candidate will be highly reliable, professional, and skilled in their approach to service delivery. Carefirst Hospice has a service area that includes Southern Indiana with the potential for providing care in multiple counties. The hospice team consists of our Medical Director, Program Director, a team of nurses, home health aides, massage therapist, pastoral care, caregivers, rehabilitation specialists (eg - PT/OT/ST), and a skilled office team available to support our clinicians in the community.
Our ultimate goal is to provide clients an experience in this season of life that honors them and their families with dignity, respect, and supplies a level of comfort that is reassuring to all parties involved.
Responsibilities
- Monitor patient’s condition and assess their needs to provide the best possible care and advice
- Make frequent visits to the patient to evaluate their condition
- Coordinates total patient care by conducting comprehensive health and psychosocial evaluation, monitoring the client's condition, promoting sound preventive practices, coordinating services and teaching and training activities
- Evaluates the effectiveness of nursing service to the patient and family
- Observe and interpret patient’s symptoms and communicate them to physicians
- Adjust and administer patient’s medication and provide treatments according to physician’s orders
- Foster a supportive and compassionate environment to care for patients and their families
- Expand knowledge and capabilities by attending educational workshops, conferences etc.
- Participates in interdisciplinary meetings, discusses with the supervisor problems concerning the patients and how they may best be handled
- Submits clinical notes within twenty-four (24) hours, and progress notes and other clinical record forms outlining the services rendered
- Obtains a medical history from the patient and/or a family member particularly as it relates to the present condition
- Conducts a physical examination of the patient, including vital signs, physical assessment, mental status, appetite and type of diet, etc.
- Evaluates the patient, family member(s) and home situation to determine what health teaching will be required
- Evaluates the patient's environment to determine what assistance will be available from family members in caring for the patient
- Explains nursing and other Agency services to patients and families as a part of planning for care
- Develops and implements the nursing care plan
- May be requested by the Program Director to fill in for other nurses who are on vacation or sick
Skills
- Proven experience as a registered nurse
- Excellent knowledge of nursing care methods and procedures
- In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times
- A team player with excellent communication and interpersonal skills
- Responsible and compassionate
- Outstanding organizational and multi-tasking skills
- Sound judgment and attention to detail
- Current CPR Certificate
- Valid nursing license
