Position Summary
The RN Case Manager will coordinate patient care for a caseload of patients that are terminally ill within Chemung, Schuyler and Steuben Counties in New York. The RN Case Manager will be the primary staff member responsible for establishing and implementing the Plan of Care for each patient on their caseload. The RN Case Manager will communicate/educate in a manner to advocate and empower and complete nursing in a fiscally responsible manner. This RN Case Manager will need to carry strong critical thinking skills, and the ability to work autonomously.
Essential Duties and Responsibilities
- Share in on call rotation
- Provides information visit and admissions as assigned
- Completing assessments of patient/family needs, development of care plans, communicate/educate in a manner to advocate and empower, and complete nursing in a fiscally responsible manner.
- Provides anticipatory guidance to patient/family as needed throughout the duration of Hospice Care.
- Makes follow up contact with family/caregiver after patient death to deal with any closure issues.
- Knows and understand state and federal regulations relating to hospice and palliative care
- Knowledge of Medicare, Medicaid, and private insurance guidelines
- Participate in interdisciplinary group meetings to update patient/family plan of care as needed
- Participate in quality assurance program and perform chart audits as assigned
- Consults with Team Managers or designee, and/or other Interdisciplinary team members before acting on changes in the patients’ health status, community referral potential, family stresses, and unanticipated nursing or medical needs
- Works with and collaborates with all members of the interdisciplinary team as indicated.
- Maintains timely documentation according to agency guidelines
- Provide supervision and evaluation of hospice and contracted homemakers, and home health aides, and LPNS.
- Participates in staff development
- Other duties as assigned by supervisor
Qualifications
Possession of a valid and current NY State professional Registered Nurse’s license. Minimum of 2 years of home care and/ or acute care experience as a professional nurse within 10 years. Current physical assessment skills. Skilled in use of nursing process. Computer Skills.
