About Us
The Community Health Worker (CHW) works collaboratively with the care team to implement a person-centered plan of care and conduct regularly scheduled homes visits. Responsible for securing, developing and preserving trusting relationships within the community, the CHW will provide consumers and caregivers general support and encouragement. The CHW will achieve this by providing health management plans and goals and coaching consumers to effectively manage their chronic health conditions and self-care.
What You Will Do:
- Assist Care Manager and/or Family Specialist with referral applications required to meet qualifications for company services
- Assist Care Manager and/or Family Specialist with home visits, home assessments, and ensure required home modifications are completed
- Collaborate with Care Manager and/or Family Specialist to implement consumer plan of care (POC)
- Identify community resources to assist consumers and caregivers; May assist consumer in completion of applications for programs in which they may be eligible
- Assist consumers in accessing health related services, overcoming barriers to obtaining needed medical care and social services
- Collect necessary documents and ensure that all required forms and standard procedures are followed
- Complete daily review of case record documentation for accuracy, completeness, compliance, and timeliness and conduct follow-ups as needed
- Perform other duties as assigned
What You Will Bring:
- 1 years of experience working with elders or people with disabilities required
- Community Health Worker (CHW) Accreditation preferred.
- Must have resided within the local community for 2 years.
- Familiarity with the resources available within the community.
- Proficiency with case management software and mobile apps
- Ability to travel (usually by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required
- Bilingual preferred
