What are the responsibilities and job description for the Nurse Practitioner position at Caregiver Support Network?
Caregiver Support Network Home Health and Hospice is looking for a part time nurse practitioner to perform hospice recertification visits. Visits may be performed in person or by face time with RN in the home. Visits will cover patients in both our Salt Lake and Kaysville offices.
