- Coordinate the data between Trella Database and Homecare Homebase CRM
- Manage the Trella database
- Run reports to help us see trends to improve performance
- Adjudicate admissions
- Dissect non-admit data
- Track critical follow-up
- Oversee the resolution reconfirmation follow-up data (at 2 weeks, 4 weeks, 8 weeks... you will learn about this)
- Organize and aggregate account data (weekly, 2x/week, monthly, etc. - for facility protocol forms, hospice patients in facilities, CPS patients in facilities, facilities with contracts)
- Numerous other administrative functions
- Preparing, compiling, and sorting documents for data entry
- Transcribing information, text, and data into the required applications, such as word processors, spreadsheets, and databases
- Responding to requests for information and accessing relevant files
- Maintaining accurate and organized filing systems
- Providing back-up assistance for other clerical positions and performing routine office support duties
- High School Diploma or GED equivalent
- 1 year of experience in an appropriate clerical setting preferred
- Current and valid Driver’s License
- Reliable transportation and valid automobile insurance coverage
- Knowledge of medical terminology
- Knowledge of CRM functions
- Demonstrate good communication, interpersonal, telephone, and documentation skills
- Demonstrate ability to multi-task.
- Associate Degree preferred
- Knowledge of Trella and Homecare Homebase
- Knowledge of Excel

- Office Hours: 8:00 am – 5:00 pm, Monday through Friday
