- Serves as a resource person regarding available health and social services and resources to the patients, caregivers, office staff, and providers.
- Follows all hospital in-patient patients through admission to discharge.
- Alerts schedulers to Hospital Discharge for follow-up with necessary TCM visits.
- Maintains patient medical records. Requests, receives, and reviews incoming and outgoing patient information correspondence and determines proper action regarding the patient's acute or post-acute stay.
- Accepts patient and/or family complaints and works to resolve them in accordance to policy.
- Responsible for having a disposition for all patients entering acute or post-acute settings for rehabilitation. Updates families and facilities regarding the disposition of the family member/resident.
- Follows all hospital in-patient patients through admission to discharge.
- Minimum one (1) year of experience in the medical field, medical office experience, or related position.
- High School Diploma or GED.
- Minimal C.E.N.A. certified or M.A. certified or equivalent work experience.
- Prefer experience with geriatric population and/or caregivers.
- Associates Degree preferred
