Responsibilities
As a Hospice Registered Nurse, your primary responsibility is to be the case manager for all patients entrusted to your care which includes assessing, planning, coordinating, implementing and evaluating the patient’s plan of treatment. In collaboration with the Hospice Medical Director, you will lead a team of individuals which may include a hospice aide, social worker, chaplain, LPN, music therapist, massage therapist and volunteers in providing all appropriate care for your patient. In this role you will be responsible for:
- Making the initial evaluation and re-evaluate the patient’s nursing needs during each visit
- Making follow up visits according to patient, family & facility needs
- Maintaining up to date patient records in an electronic medical record system so that problems, plans, actions and goals are accurately and clearly stated, and changes are reflected as they occur
- Communicating significant findings, problems, and changes in condition or environment to the supervisor, physician, facility and/or other personnel involved with patient care
- Carrying out the mission and values of hospice
- Maintaining professional boundaries
Required Qualifications
- Graduate of an accredited school of nursing
- Current RN license in state
- One (1) year of RN experience
- Complete required work daily
- Cope with work involving ill, disabled, and terminal patients
- Sit, stand, bend, lift, and move intermittently
- Ability to lift at least 25lbs and bear the weight of an average adult effectively
- Excellent observation, verbal and written communication skills
- Problem solving skills and basic math skills
Desired Qualifications
- Bachelor’s degree in skilled nursing (BSN)
- Hospice and/or Home Health experience
- Familiarity with Home Care Home Base (HCHB)

- Main Service Area: Jackson and surrounding areas
Hours
- 5:00 p.m. to 8:00 a.m.
- On call for 7 days then off the next 7 days
