What are the responsibilities and job description for the Care Coordinator position at CareLink?
CareLink, the Area Agency on Aging for Central Arkansas, is a nonprofit agency providing resources for older people and their families. We need your support to serve the growing population of older people in Faulkner, Lonoke, Monroe, Prairie, Pulaski, and Saline Counties. MINIMAL QUALIFICATIONS
- Be licensed in the state of Arkansas as a social worker LCSW (Licensed Clinical Social Worker), an RN (Registered Nurse) or an LPN (Licensed Practical Nurse), or
- Have a bachelor’s degree from an accredited institution in either business administration degree health and human services degree, or
- Have an associate degree from an accredited institution with five years of experience in Community services and Case Management, and
- Have two years of experience in the delivery of human services, including without limitation having performed satisfactorily as a case manager for a period of two years. (Experience must be within the past three years).
- Reliable transportation with a valid AR driver’s license and auto liability insurance with increased coverage limits
KNOWLEDGE, SKILLS, AND ABILITIES
- Knowledge of governmental resources and programs, including but not limited to Medicaid and Medicare eligibility requirements, including Medicare Part D counseling.
- Ability to drive a car to clients’ homes within the six-county service area.
- Ability to objectively interview and obtain accurate information.
- Ability to evaluate problems and develop a person-centered service plan with the client’s input and approval.
- Ability to collect and enter data and prepare reports.
- Ability to accurately prepare forms and reports on time.
- Ability to network with other agencies to coordinate services.
DUTIES AND RESPONSIBILITIES
- Receives referrals and contacts client/caregiver to gather initial information.
- Schedules home visits with client/caregiver as needed.
- Performs initial interview/assessment to determine needs.
- Conducts reassessment as required by funding source.
- Assesses eligibility and presents service options available to the client/caregiver.
- Assists client as needed in completing application forms for services needed.
- Contacts other agencies on behalf of the client as appropriate
- Provides follow-up services.
- Documents all contacts for the file, e.g., client, family, outside agency, etc., and forwards copies for billing of services.
- Provides appropriate documentation for the billing of service units.
- Obtains physician signature for Medicaid billing.
- Secures signed release of information form before contacting any outside agency on behalf of the client.
- Provides summary reports.
- Gathers information from callers and prepares referrals and sends them to appropriate staff or departments.
- Ensures that the customer has been helped and has reached the correct person to limit the need for return calls.
- Provides information about aging services, public benefits, and other resources to clients’ caregivers, other agencies, and the public.
- Promotes agency services and programs, and mail brochures.
- Helps gather information on resources in the community.
- Stay well informed of changes in agency services, benefits, programs that may benefit the client or family caregiver.
- Attends staff meetings and training.
- Other duties as assigned.
ADDITIONAL INFORMATION
This is a full-time position with 37.5 hours/week. Eleven paid holidays. Bi-weekly compensation, mileage reimbursement, employee/family health insurance, 401K retirement plan, sick, and annual leave.
