About the Role :
The Hospice Medical Social Worker plays a crucial role in providing emotional and psychological support to patients and their families during end-of-life care. This position involves assessing the social, emotional, and financial needs of patients, ensuring they receive comprehensive support tailored to their unique circumstances. The MSW will collaborate with a multidisciplinary team to develop and implement care plans that address both the medical and psychosocial aspects of hospice care. By facilitating communication between patients, families, and healthcare providers, the MSW helps to create a compassionate environment that honors the wishes and dignity of those facing terminal illness. Ultimately, the goal of this role is to enhance the quality of life for patients and their loved ones during a challenging time.
Minimum Qualifications :
- Master's degree in Social Work (MSW) from an accredited program.
- Has 6 months of social work experience in hospice home care, or skilled home health.
- Ability to travel to patients homes for visits
- Must be board certified in the State of California.
