About the Role:
As a Registered Nurse in the Home Health Per-Diem position, you will play a crucial role in delivering high-quality, patient-centered care to individuals in their homes. Your primary responsibility will be to assess, plan, implement, and evaluate nursing care for patients with varying health conditions, ensuring their comfort and well-being. You will collaborate with interdisciplinary teams to develop personalized care plans that address the unique needs of each patient. This role requires a strong commitment to patient education, empowering individuals and their families to manage their health effectively. Minimum Qualifications:
- Current and valid Registered Nurse (RN) license in the state of practice.
- Bachelor of Science in Nursing (BSN) or an Associate Degree in Nursing (ADN) with relevant experience.
- Basic Life Support (BLS) certification.
Preferred Qualifications:
- Experience in home health care or community health nursing.
- Certification in a specialty area such as gerontology or palliative care.
- Proficiency in electronic health record (EHR) systems.
Responsibilities:
- Conduct comprehensive assessments of patients' health status and needs in their home environment.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Administer medications, treatments, and therapies as prescribed, while monitoring patients for any adverse reactions.
- Provide education and support to patients and their families regarding health management and disease prevention.
- Document all patient interactions, care provided, and changes in condition accurately and timely
- Coordinate and monitor patient care across multiple healthcare providers and settings
- Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure coordinated and effective care delivery
- Provide education and support to patients and their families regarding treatment options, resources, and available support services
- Advocate for patients' rights and ensure their needs are met within the healthcare system
- Maintain accurate and up-to-date documentation of patient interactions, care plans, and progress notes
- Monitor patient progress and make adjustments to care plans as needed
- Ensure compliance with HIPAA regulations and maintain patient confidentiality
Experience:
- Previous experience in case management or a related field is preferred
- Knowledge of healthcare systems, including acute care, home health, assisted living, and nursing home settings
- Strong leadership skills with the ability to supervise and coordinate the work of others
- Excellent communication skills to effectively collaborate with interdisciplinary teams and advocate for patients' needs
- Familiarity with anatomy and medical terminology is beneficial
