Duties and Responsibilities:
- Routinely incorporates patient and family education into patient care
- Identifies patient, patient family and community education needs
- Ensures infection control and safety measures are implemented per policy for patients and communicated to appropriate caregivers and family members in a timely manner
- Utilize skills in patient/client evaluation and treatment, professional consultations to achieve maximum patient/client independence in accordance with individual‘s functional level and potential
- Communicates with Rehab Program Manager, Patient Care Manager, Clinical
- Coordinator and appropriate staff in order to plan total patient and family care
- Records reflect the family-oriented approach
- Develop and implement treatment and discharge plans based on results modifying as necessary to meet resident/patient goals
- Provide documentation of patient services according to departmental policies
- Assume case management and patient advocacy roles
- Develops and maintains positive relationships with residents, patients, families, physicians, and staff
Skills/Qualifications:
- Two years’ experience in an acute care or rehabilitation setting, home health preferred
- Certificate of Clinical Competence for Speech–Language Pathology (CCC-SLP)
