Field Case Manager (Social Worker/Counselor)

CareNational is looking for an experienced Field Case Manager (Social Worker/Counselor) in Illinois ! This is a contract position and ideal candidates will have at least 2 years of recent experience as a Social Worker or Counselor.
Pay Details:
- Pay Range: $34.11 / Hour
Work Setting:
- Hybrid
Position Details:
- Contract Length: 5-6 months
- Location: Springfield, IL (Sourcing for Macon County with a focus on areas around Decatur.)
- Monday-Friday 8:00am-5:00pm
Requirements:
- Bachelor's degree in social services, human services or
behavioral health. - Minimum of 2 years of experience in behavioral health or
social services field or human services. - Must have 2 years of experience in case management
- Willing and able to travel using your own vehicle 50-75%
of their time to meet with members face to face in Jefferson County and surrounding areas (Must have reliable
transportation and a drivers license)
