- Shifts: 7a-3p, 3p-11p, 11p-7a
- Identify special resident problems and changes in condition and report them immediately to supervisor
- Bathes, neatly dresses and undresses residents
- Grooms resident including shaving, hair and fingernail car
- Accurately records percentage of resident food and fluid intake
- Feeds resident according to care plan(s)
- Monitors meal trays
- Answers call lights timely
- Delivers and removes resident meal trays
- Pulls up side rails and rolls down bed
- Changes incontinent residents
- Takes residents to bathroom every 2 hours or as requested
- Completes tub baths, showers or bed baths
- Turns and repositions all bed and chair residents every 2 hours including but not limited to pulling up in bed
- Empties and records all urinary drainage units
- Assists nurse with treatment procedures
- Changes bed linens on occupied and unoccupied beds
- Takes, records and reports weights of residents
- Inspects resident skin condition with daily care and reports to the supervisor
- Minimum 10th grade education. High School Diploma or GED preferred
- Must be at least 16 years old
- Must be a Certified Nursing Assistant in the state of Massachusetts and have an active license
