CareNational is looking for an experienced Field RN Case Manager in Bureau County, IL! This is a contract position and ideal candidates will have at least 3-5 years of recent experience as a Field RN Case Manager.
Pay Details
- Pay Range: $35.86 / Hour
- Hybrid (50-75% travel throughout Bureau County)
- Contract Length: 3 months
- Location: Bureau County, IL
- M-F 8-5 CST
- RN, LCSW, or LCPC with current unrestricted IL state licensure REQUIRED
- Requires unrestricted driver's license and car.
- Minimum 2-3 years Care Management, discharge planning and/or home health care coordination experience preferred
- Minimum 3-5 years clinical practical experience preferred
- Excellent analytical and problem-solving skills
- Effective communications, organizational, and interpersonal skills.
- Demonstrates proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as some special proprietary applications
- Effective computer skills including navigating multiple systems and keyboarding
