CareOne Home Health is seeking an Intake Coordinator for our growing home health business in Salida, CA. The Intake Assistant assists in processing home health referrals and coordinating amongst the clinical staff, referral source, and marketing team to ensure our patients are properly entered into our software, scheduled for a visit, and all stakeholders are informed of patient status.

Requirements:
- 1 year home health intake experience, required
- Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
- Experience with Kinnser (WellSky) EMR preferred
- Experience with Allscripts preferred
- Bilingual Spanish Speak, read and write preferred
Primary responsibilities include:
- Assists with answering phones.
- Receives intake information for patients and enters the appropriate information into the computer.
- Completes intake forms and routes them appropriately for admission approval.
- Assists in the scheduling of appropriately skilled person to fill the case, inputs scheduling information on the computer and continues to assist in the coordination of the scheduling.
- May assist with scheduling replacement workers as needed and input scheduling information in the Electronic Medical Record.
- Assists in communication with patients and families.
- Takes telephone referral information if no nursing staff is available and passes referrals as soon as possible.
- Handles matters related to staff and the patient they serve, with the exception of issues that require professional/clinical judgment.
- Regularly communicates with referral sources and contract agencies to maintain smooth relations and obtain needed information.
- Documents and reports changes in patient status.
- Verifies information including physician at Start of Care.
- Scans and uploads referral documents.
