Responsibilities
- Serve as the primary point of contact throughout the admissions process, ensuring seamless coordination with other team members and marketing teams.
- Communicate effectively with patients, family members, case managers, healthcare providers, and co-workers to provide personalized patient education on home healthcare experiences.
Key Requirements:
- ~2 years of LVN experience in home health preferred
- ~ Valid LVN license in California
- ~ Proficient in Microsoft Office software (Word, Excel, PowerPoint, Outlook)
- ~ Experience with Kinnser (WellSky) EMR or Allscripts preferred
- ~ Current driver's license and valid auto insurance
- ~ BLS/CPR certification
