Job Description
As a Long-Term Care Specialist, you will play a vital role in ensuring the highest quality of care for our residents.
Responsibilities:
- Assist with the completion of the Minimum Data Set (MDS) and Comprehensive Assessment Allegations (CAAs).
- Contribute to the development of individualized resident care plans, identifying resident needs and projected outcomes.
- Work collaboratively with the care team to ensure accurate and efficient completion of MDS items, CAAs, and care plans.
- Participate in resident care plan meetings and contribute to discussions regarding resident care and services.
Qualifications and Requirements:
- RN licensure in the state of practice, with experience in PPS and OBRA assessments.
- Previous experience in hospitals, long-term facilities, or other healthcare-related settings.
- Familiarity with general, rehabilitative, and restorative nursing practices, as well as medical procedures, laws, regulations, and guidelines governing long-term care.
- Proficiency in Windows applications and computer experience, preferably with Accu Med.
- Proven ability to complete MDS and care plans accurately and efficiently.
- 1-3 years of MDS experience or RN with LTC experience that can be trained for an assessment role.
- Ability to work independently and flexibly with scheduling for month-end close.
- High attention to detail and accuracy in completing assessments on time.
