Assist Director with assessments interviews family meetings and followup.
Work with interdisciplinary group to reinforce discharge planning and care initiatives.
Interact with all disciplines to ensure all aspects of care are identified and addressed.
Work with outside agencies for proper care protocols once discharge has been set.
Job Type: Full-time
Pay: $25.00 - $30.00 per hour
Expected hours: 40 per week
