CarePartners Rehabilitation Hospital is an 80-bed inpatient facility for acute rehabilitation located in Asheville, NC.
CarePartners brings together four services – rehabilitation, home health, adult care and hospice – to offer our patients a full continuum of post-acute care.
Job Requirements
Home Health experience, w / 2 years minimum clinical experience
BLS certification required.
Schedule Information
One weekend on call every 4-6 weeks (day shift : visits / evals, FA’s or admits).
Patient Ratios – 5-6 home visits per day.
Unit Specific Information
Manage caseload for PT and PTA in that area, minimum visits of 5.6 visit equivalents per day, supervising PTA and managing all patient PT needs including communication w / MD office.
