About This Role
The Primary Case Manager RN Home Health will be an integral part of our rehabilitation team, working closely with patients, families, and healthcare providers to deliver exceptional care. Key responsibilities include:
- Providing education and support to patients and families, promoting active participation in care planning and goal setting.
- Developing and implementing individualized care plans that address patient needs, preferences, and goals.
- Collaborating with the interdisciplinary care team to ensure comprehensive support for patients and families.
- Documenting patient progress and adjusting care plans as needed to optimize outcomes.
Requirements
- BLS certification within 30 days of hire.
- Valid driver's license within 2 years of employment.
- Active Registered Nurse licensure within 2 years.
- Associate Degree required.
