We are seeking a skilled Registered Nurse to provide high-quality, patient-centered care in the home setting. As a member of our interdisciplinary team, you will be responsible for assessing patients' needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure seamless transitions in care.
Key Responsibilities:
- Conduct comprehensive assessments of patients' physical, psychosocial, and environmental needs, identifying areas for improvement and developing targeted interventions.
- Develop and implement personalized care plans that address patients' goals and outcomes, in collaboration with physicians, therapists, and other healthcare professionals.
- Provide skilled nursing care, including wound care, medication management, and education, to patients in their homes.
- Collaborate with other healthcare professionals, including social workers, therapists, and home health aides, to ensure comprehensive care and support for patients and families.
- Communicate effectively with patients, families, and healthcare professionals through verbal and written reports, phone calls, and electronic communication.
- Participate in on-call rotation and emergency call as needed, providing timely and effective response to patients' needs.
