Job Summary
The Home Health Case Manager- PT will work in a dynamic environment where diversity and inclusion thrive. This individual will collaborate with colleagues to ensure high-quality, efficient care for all patients.
Key Responsibilities:
- Collaborate with the Care Management department, service line, and interdisciplinary care team to promote quality-driven best practices across the continuum of hospitalization.
- Maximize positive financial outcomes for designated patient populations by intervening at key points in the disease process and promoting health compliance and preventing readmissions.
- Provide designated leadership functions for the Care Management Department.
Requirements:
- RN with a Bachelor of Science in Nursing or related field (BSN) or Bachelor degree in a related field.
- Current licensure as a Registered Nurse with the North Carolina Board of Nursing or a compact state as defined by the North Carolina Board of Nursing.
- Minimum of Five years recent clinical nursing experience in a relevant practice area.
- Bi-lingual.
