Job Summary
CarePartners is seeking a Registered Nurse to lead our Home Health team in providing high-quality, patient-centered care. As a Post-Acute Care Manager, you will collaborate with colleagues to ensure seamless transitions between hospitalization and post-acute care.
About the Role
This role requires a strong clinical background, excellent communication skills, and the ability to lead interdisciplinary teams. You will work closely with patients, families, and healthcare providers to develop personalized care plans that promote optimal health outcomes.
Key Responsibilities
- Collaborate with Care Management department, service line, and interdisciplinary care team to ensure quality-driven best practices across the continuum of hospitalization
- Maximize positive financial outcomes for designated patient populations through targeted interventions and post-discharge follow-up phone calls
- Provide leadership functions for the Care Management Department
Requirements
- Current licensure as Registered Nurse with the North Carolina Board of Nursing or a compact state
- Maintains nationally recognized certification in Case Management (ACM or CCM, or ANA approved certification)
- Minimum 5 years recent clinical nursing experience in relevant practice area
- Basic Cardiac Life Support must be obtained within 7 days of employment start date
