Job Summary
The Primary Case Manager RN Home Health will work closely with our rehabilitation team to ensure patient-centered care by managing rehabilitative care to achieve optimum outcomes.
- You will provide program orientation to patients/families/caregivers, including the case manager's role, rehabilitation philosophy, and continued stay/discharge criteria.
- Act as the coordinator of patient/family/caregiver education, promoting participation in team discussions related to plans, goals, and status through family conferences and other interactions.
- Ensure implementation of the patient's treatment plan that supports their strengths, abilities, needs, and preferences, facilitating patient involvement throughout the rehabilitation process.
- Document findings from the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient/family/caregiver.
- Promote consistent, positive patient interactions advancing the agenda of unparalleled patient service.
Requirements
- BLS certification must be obtained within 30 days of employment start date.
- A valid driver's license must be obtained within 2 years of employment start date.
- Registered Nurse licensure must be obtained within 2 years of employment start date.
- An Associate Degree is required.
