The licensed vocational nurse is responsible for providing direct patient care under the supervision of a registered nurse. LVN Field Nurse and Intake Job Responsibilities and Duties
- Provide direct patient care, related to end-of-life care
- Implement current nursing practices following plan of care
- Provide accurate and timely documentation of patient services
- Asses and provide patient and family/caregiver education
- Participated in coordination of hospice services
- Uses and prepares equipment and supplies effectively
- Provides appropriate pain/symptom management
- Participates in personal, professional growth and development
- Perform other duties assigned by RN
- Answer multiple-phone calls
- Screen and triage patient calls, and take messages.
- Coordinate services between staff and patient.
- Completing projects or special assignments when requested.
- Copy and assemble reports or documents as needed.
- Order medical equipment and supplies as requested
- Obtaining insurance authorizations
Qualifications:
-1 years of hospice experience. (Required)
-BLS Certification
-LPN
-Excellent written and verbal communication skills.
-Understands HIPAA
-Good problem-solving skills, able to multi-task and prioritize work quickly.
-Work requires continual attention to detail, establishing priorities and meeting deadlines.

Pay: $28.00 - $45.00 per hour
