About the Role : As a Case Manager, youll play a crucial role in connecting individuals with meaningful employment opportunities while providing comprehensive case management support. This dynamic position requires travel throughout King and Snohomish counties and involves close collaboration with clients, employers, and community partners.

Employment Planning : Develop detailed Employment Plans with clients and their stakeholder teams, outlining specific outcomes, strategies, and support activities.Job Development : Source and secure employment opportunities for individuals who are low-income and / or have disabilities.Career Coaching : Guide clients through resume writing, job search strategies, and interview preparation.Case Management : Maintain detailed documentation in multiple systems (DSHS eJAS, HMIS, and SetWorks) while ensuring compliance with all organizational and funder requirements.Team Collaboration : Actively participate in team meetings and planning sessions.Additional Tasks : Perform other duties as needed.Requirements / Qualifications :
Valid WA State drivers license and proof of valid automobile insuranceReliable transportation and willingness to travel extensively across King and Snohomish countiesCurrent First Aid / CPR certification preferred or willingness to obtain within 30 days of hireCompliance with form I-9 and Immigration and Naturalization Services requirementsAbility to work independently and adapt to a flexible scheduleSuccessful completion of a WA State criminal history background checkPreferred Experience :
Bachelors degree in case management, human services, or related field2-3 years of case management experienceProficiency with computers and college-level writing skillsKnowledge of case management, human services, and customized employment practicesExperience with job development and recruitment practicesBilingual skills (Spanish / English)
