Family Peer Advocate (FPA) for CFTSS Program
Salary Range: $20-$25/hr Location: Remote plus travel
Essential Duties and Responsibilities:
The role of a Family Peer Advocate is to support families whose child is experiencing developmental delays, mental health issues or other challenges. This can be accomplished through various methods such as phone calls, giving access to resources, researching solutions, attending meetings and building connections. This position must obtain certification within 18 months and requires 1,000 hours of ___ and numerous trainings, all of which will be provided by CARES. The pay scale is $20/hr through 250 hours, $21/hr up to 500 hours, $22/hr up to 750 hours and $25 when certified.
- Build connections with families through numerous avenues such as offering skill-building, frequent interactions, referrals, etc. following the directive of the treatment plan.
- Ensure that the child and family are using accessible resources and services.
- Complete all documentation after each interaction and on an ongoing basis.
- Provide goals when families are in need and extend a helpful hand to track progress.
- Initiate and review consent forms for submission. Correct errors and omissions as necessary.
- Perform related duties as assigned by supervisor.
- Weekly meetings with supervisor.
- Maintain compliance with all company policies and procedures.
Education and/or Work Experience Requirements:
- Must have the lived experience of being the primary caretaker of a child who minimally has an IEP(Individualized Education Program) to severe impairment that was diagnosed prior to turning 21 years old.
- This position is remote. However, in the event that the family has an in-person meeting regarding their child and asks that you join, we would expect you to attend in-person as well.
- Excellent verbal and written communication skills, including ability to effectively communicate with internal and external customers.
- Computer proficiency (MS Office Word, Excel and Outlook).
- Experience with EHR(Electronic Health Reords) is a plus.
- Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service.
- Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.
- High school diploma or GED required.
Physical Requirements:
- Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
- Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
- Must be able to lift and carry up to 35 lbs.
- Must be able to talk, listen and speak clearly.
