ODP Coordinator
Position: ODP Coordinator
Type: Full-Time
Schedule: Monday - Friday, 8:00 AM - 4:00 PM
Location: Suffern, NY

Key Responsibilities:
Oversee and coordinate services for individuals with developmental disabilities, ensuring they receive the services and support they need to thrive in their communities.
Develop and maintain care plans in collaboration with individuals, families, DSP's and healthcare providers to implement individualized care plans that meet the unique needs of each person.
Schedule and manage DSP assignments ensuring that all services comply with ODP regulations and agency policies, assist with case management, and facilitate communication between all parties involved
Monitor client progress and adjust care plans as needed.
Address client and DSP concerns and resolve issues in a timely and professional manner.
Maintain accurate and up-to-date records of care services provided.
Work efficiently in a fast-paced environment, managing multiple tasks and priorities effectively.
Qualifications:
High school diploma or equivalent required; Associate’s or Bachelor’s degree in healthcare, social work, or a related field a plus.
Previous experience working with IDD individuals or in home care coordination is highly desirable.
Strong organizational skills with a keen attention to detail.
Ability to work quickly and effectively under pressure.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Proficiency in Microsoft Office and other relevant software.
Must be able to work Mon-Fri 8am-4pm in Suffern office.
