Hospice Aide / CNA Position

Requirements
- Under the direction of a registered nurse, provide direct patient care services as established in the Plan of Care.
- Report patient encounters and conditions to the Executive Director or designee, following the Care Management process.
- Assist with ADLs (Activities of Daily Living) such as mobility, transfers, grooming, bathing, dressing, eating, and toileting.
- Perform incidental household services essential to patient health care, preventing or postponing institutionalization.
- Participate in QAPI (Quality Assessment & Performance Improvement) processes to identify problems and report administrator daily or as needed.
- Maintain reliable transportation and current licensure requirements.
Licensure Requirements
- Successful completion of a competency evaluation prior to patient care.
- Current CPR certification requirements.
- Current driver's license, vehicle insurance, and access to dependable transportation.
Skill Requirements
- Maturity and ability to handle hospice aide position demands.
- Ability to read, write, and carry out directions promptly and accurately.
