Your Role in Compassionate Care
As a Hospice RN Case Manager, you will be responsible for delivering top-notch care to our patients, working closely with them and their families to create personalized plans that cater to their unique needs.
Responsibilities:
- Conduct thorough initial assessments to determine eligibility for hospice services within 48 hours of referral.
- Continuously evaluate patient needs, reassessing every 15 days or as needed.
- Develop and coordinate comprehensive plans of care, documenting patient responses and progress.
- Collaborate daily with patients, families, attending physicians, and interdisciplinary teams to deliver seamless care.
Requirements
To excel in this role, you will need:
- One year of clinical experience.
- Current RN licensure in the state of practice, with one year of clinical experience.
- Current CPR Certification.
- A valid driver's license, vehicle insurance, and reliable transportation or access to public transportation.
