As a RN Case Manager, you will be responsible for developing trusting relationships with patients and their families. You will make initial nursing evaluations, identify patient needs, and initiate and coordinate plans of care.
Responsibilities

- Coordinates with healthcare teams to ensure seamless delivery of care.
