Caretenders Hospice is committed to delivering high-quality, patient-centered care to individuals and families facing life-limiting illnesses. As a Hospice Case Manager RN, you will play a critical role in developing and implementing personalized Plans of Care for our patients.
You will work collaboratively with our interdisciplinary team to ensure comprehensive care and support, and provide education and support to patients and families on end-of-life care and related topics. The ideal candidate will possess strong clinical skills, excellent communication and interpersonal skills, and a passion for delivering exceptional care.
Key Responsibilities:
- Develop and implement personalized Plans of Care for Hospice patients
- Evaluate personal and professional performance to increase productivity and quality of care delivered
- Collaborate with interdisciplinary teams to ensure comprehensive care and support
- Provide education and support to patients and families on end-of-life care and related topics
Requirements:
- Clinical experience preferred
- Current RN licensure in good standing
- CPR certification required
