Job Description
We are seeking a Registered Nurse (RN) to join our Home Health team in Indianapolis. As a skilled nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with physicians and other healthcare professionals to ensure seamless coordination of care.
Key Responsibilities
- Provide comprehensive nursing care to patients in their homes, including assessing their physical, emotional, and social needs.
- Develop and implement individualized care plans that meet the unique needs and goals of each patient.
- Collaborate with interdisciplinary teams to coordinate care and services, including therapy, medication management, and equipment provision.
- Communicate effectively with patients, families, and healthcare teams to ensure understanding and cooperation.
Requirements
- Current RN licensure in state of practice
