Job Summary
As a Home Health RN, you will be responsible for providing comprehensive nursing care to patients in their homes. Your duties will include:
Duties and Responsibilities
- Assess patients' physical, emotional, and social needs, and develop individualized care plans that meet their unique needs and goals.
- Implement care plans, including administering medications, providing wound care, and promoting independence.
- Collaborate with interdisciplinary teams to coordinate care and services, including therapy, medication management, and equipment provision.
- Communicate effectively with patients, families, and healthcare teams to ensure understanding and cooperation.
Qualifications
- Current RN licensure in state of practice
- Ability to work independently and collaboratively as part of a team
