We are seeking a qualified Patient Care Manager to join our team and lead our clinical services efforts. As a Patient Care Manager, you will be responsible for the supervision and coordination of clinical services, including patient assessments, care planning, and service delivery.
About the Role:
- Develop and implement plans of care that meet the unique needs and goals of each patient.
- Supervise and coordinate the work of clinical staff, including RNs, LPNs, and HHAs.
- Collaborate with other departments to ensure seamless patient flow and satisfaction.
- Maintain accurate and up-to-date records of patient care and services provided.
What We're Looking For:
- Current RN licensure in state of practice.
- Current CPR certification.
- At least 3 years of experience in home health or a related field.
