The Physical Therapist in Home Health plays a vital role in assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, they develop a treatment plan to improve and enhance the patient's well-being.
The key responsibilities include:
- Evaluating patient function and physical therapy needs, assisting physicians in creating therapy plans.
- Monitoring patient responses to treatment and reporting any changes to supervising nurses or physicians.
- Treating patients to alleviate pain, restore function, and maintain optimal functioning using accepted standards of practice.
- Informing the healthcare team, patients, and caregivers about treatment plans, functional limitations, and progress towards goals.
Key qualifications for this position include:
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- A valid driver's license, vehicle insurance, and access to a reliable vehicle.
