Job Responsibilities:
The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
- Evaluate patients' functional status and physical therapy needs, assisting physicians in developing therapy plans of care.
- Observe and report patients' responses to treatment and any changes in their condition to supervising nurses and/or physicians.
- Administer treatments to relieve pain, develop and restore function, and/or maintain maximum functioning according to accepted standards of practice.
- Inform and instruct participating members of the healthcare team, patients, and family/caregivers regarding plans of care, functional limitations, and progress toward goals.
Licensure Requirements:
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- A valid driver's license and vehicle insurance, with access to a dependable vehicle.
