Our Medical Records Clerk organize, file, and maintain client medical records including paper and electronic files in accordance with agency policy. Ensures medical records are compliant with State, Federal and HIPAA regulations. Assists in retrieving medical records from medical providers and responds to internal and external medical record requests from various sources including healthcare providers.
Education/Experience:
- Education generally equivalent to a high school diploma is required.
- One to three years of verifiable work history including experience in medical records.
Responsibilities:
- Gathers patient information by collecting demographic information from a variety of sources; interacting with registration areas and physicians' offices; retrieving information from electronic health record systems.
- Review patient records for accuracy, identifying and correcting errors.
- Ensures clients have up to date and accurate medical records and/or release of information consent forms.
- Collects and records medical, social and reimbursement data pertinent to patient admission to initiate home care.
- Monitors insurance changes and request authorizations for continuity of care, including extensions and re- certifications.
- Collaborates with the nursing department when reviewing and processing physician orders for instructions on patient's plan of care and prior to state submittal.
- Communicate with clients, doctors, and in-house nurses for coordinated and on-going processing updates.
- Conduct phone support to clients and care workers to provide medical records and processing updates.
- Maintain strict confidentiality of patient medical information.
- Serves and protects the home health organization by adhering to professional standards, home health policies and procedures, federal, state, and local requirements, and DHS standards.
Competencies:
- Effective oral and written communications and presentation skills are essential, as are interpersonal skills.
- Understanding of basic medical terms and abbreviations.
- Ability to accurately record and manage sensitive medical information.
- Ability to work with frequent, established deadlines is required.
- Ability to develop, plan, and implement short- and long-range goals is required.
- Ability to work effectively in a team environment is required.
- Computer literacy including MS Office 365, Google Suite, and Electronic Health Record systems.
