Job Description
We are seeking an Home Health Case Manager to join our team! You will work alongside other medical professionals to provide exceptional care to patients.
Responsibilities:
- Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate.
- Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
- Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
- Participate in standardized health care programs and seminars
- Contact the discipline assigned to the patient to follow up on their visits.
- Works collaboratively with nursing personnel and case managers to ensure optimal care throughout the health care continuum. As necessary makes home visits.
- Meets monthly with other team members. Reports on the patient's progress and makes recommendations on changes to the plan of care to ensure patient compliance and positive patient outcomes.
- Conducts scheduled follow-up communications with patients for adherence and tracking.
- Assists Clinical Director/Director of Patient Care Services in the planning, implementation and evaluation of in-service and continuing education programs.
- The Case Manager provides an efficient, coordinated approach to clinical management, with an emphasis on discharge planning, care coordination and utilization management activities for select patient populations across the continuum
- The essential functions of the Case Manager require constant collaboration with the Social Workers, Nursing staff, Physical Therapy Contractors, Medical Staff to coordinate the care of patients in a proactive manner of programs and services
- Performs other duties and activities as delegated by the Clinical Director/Director of Patient Care Services.
Qualifications:
- LVN License in the State of California preferred.
- Must have a strong clinical assessment and critical thinking skills necessary to provide utilization review/discharge planning services appropriate to patients with complex medical, emotional and social needs.
Must have the ability to work in a high volume caseload environment and deal effectively with rapidly changing priorities.
- Ability to track outcomes and report findings.
Able to problem solve effectively.
Ability to use clinical knowledge to identify potential quality issues, delays in service, post-acute care needs required.
- Ability to case manage smoothly and increase patient/physician satisfaction while staying within guidelines.
- Ability to negotiate orders with the physicians in order to assign alternate levels of care.
- Previous experience in patient care or other medical fields
- Knowledge of medical terminology
- Ability to build rapport with patients
- Excellent written and verbal communication skills
- Must be able to function in a fast paced environment and possess good people skills.
- Excellent attendance is mandatory.
- We prefer experience but we are willing to train
- Strong leadership qualities
