What are the responsibilities and job description for the Hospice Liaison position at Carewell Hospice?
- Hospice Service Representative experience a MUST.
We are looking for an experienced Full- Time sales representative with Hospice Sales Experience, Skilled Nursing and or Assisted Living experience. Ideal candidate will already have established professional relationships with physicians, discharge planners, social workers, facilities and health care decision makers.
Reporting to the Administrator
ESSENTIAL JOB DUTIES
- Grow business through new referral sources.
- Coordinate with patients and their families for care conferences as needed and assist them with all admission paperwork as needed.
- Initiation of leads through relationships with qualified sources and contact with patients and families
- Identify referral opportunities through market intelligence and territory management data
- Develop and implement special events and presentations to assist in maximizing fair market share
- Facilitate patient transition to company services
- Meet and exceed company established targets
- Utilize company database to input all sales activities on a daily/weekly basis. Create weekly sales report reflecting the week's activities and referrals
- Valid driver’s license, and auto insurance, reliable/well maintained transportation.
- Conducts self in an honest, professional and ethical manner
