What are the responsibilities and job description for the Intake Manager position at Caring Angels Home Health?
The Intake Manager is responsible for the overall coordination of agency referrals and scheduling for Medicare and non-Medicare clients under the direct supervision of the clinical supervisory staff. The Intake Manager is a resource person for referral sources and works closely with other members of the team to ensure adequate coordination and transition occurs to home health services.
ESSENTIAL DUTIES and RESPONSIBILITIES :
The essential functions include, but are not limited to the following:
- Intake:
- Serves as the first line of customer service for referral sources and clients (patients or their families)
- Receives and documents patient referrals in a referral tracker and the EMR system
- Gathers, calls and confirms the accuracy of appropriate information (from the referring facility/physician) to assist the clinical team in scheduling an evaluation visit
- Coordinates with other departments for insurance verification and payer authorization as needed
- Communicates and follows up with clinical management regarding pending admissions
- Creates initial SOC visit, associated tasks and assigns to the appropriate clinician
- Performs other in-take related duties as assigned
- Patient Charts:
- Prepare new patient charts and work with the clinical team to ensure all the information is uploaded
- Office Tasks:
- Maintain medical supplies inventory & input medical supplies orders
- Maintain office supplies & input office supplies orders
- Assist with scheduling, payroll/timesheets reconciliation, onboarding, orders as needed if some of the other staff is not available
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, and ABILITIES)
- Experience in the home health care industry
- Proficiency with Microsoft Office suite and overall computer skills
- Excellent written and verbal skills
- Strong quantitative and analytic skills
- Ability to adapt to a rapidly changing working environment
- Excellent interpersonal skills, and an aggressive approach to excellence
- Open to dynamic change and ability to thrive in such an environment
- Initiative to take on new tasks as necessary
- Education equivalent to a High school diploma, college degree preferred
- Willingness and ability to work effectively with members of other departments
