Position Overview
Caring Companions is seeking a proactive, highly organized, and compassionate Client Care Manager to lead our dedicated caregivers and ensure exceptional client support. In this pivotal role, you will oversee caregiver scheduling, supervision, and training while ensuring seamless care delivery. You’ll also manage after-hours Care Coordinators to maintain uninterrupted service and step in when necessary to address urgent needs. Additionally, the Client Care Manager will conduct client intakes and follow up on client leads to ensure personalized care and client satisfaction.
The ideal candidate will possess caregiving experience, strong communication skills, and the ability to multitask in a fast-paced environment. This role requires a solution-oriented approach and a passion for creating positive outcomes for caregivers and clients alike.
Work Schedule
- In-Office: Monday, Tuesday, Thursday, Friday (8 AM–5 PM)
- Optional Work from Home Day: Wednesday (8 AM - 5 PM)
- On-Call After-Hours: Time spent training new after-hours Care Coordinators, ongoing support given to after-hours Care Coordinators, and cover their duties when needed
Key Responsibilities
1. Scheduling and Shift Management
- Take full responsibility for caregiver scheduling, ensuring every shift is covered.
- Address last-minute call-outs by coordinating replacements or personally covering shifts.
- Maintain an accurate, up-to-date scheduling system and communicate changes promptly to caregivers and clients.
- Anticipate staffing gaps and use creative problem-solving to prevent service disruptions.
2. Care Plan Updates and Quality Assurance
- Review and update client care plans quarterly, incorporating caregiver feedback and supervisory observations.
- Conduct in-home visits to assess care quality and recommend adjustments to care plans.
- Clearly communicate updates to caregivers, clients, and families, ensuring transparency.
3. Caregiver Training
- Lead weekly 4-hour hands-on training sessions for new caregivers, covering skills, agency policies, and care standards. (Lead Caregiver may also lead the hands-on training)
- Provide additional in-home training and support for caregivers when needed.
4. On-Call Responsibilities
- Supervise after-hours Care Coordinators during evenings and weekends, providing support when necessary.
- Step in to handle after-hours scheduling emergencies, open shifts, or client needs if the coordinator is unavailable.
- Hire and train after-hours Care Coordinators to maintain a strong support system.
5. Caregiver Supervision and Support
- Provide consistent supervision, guidance, and mentorship to caregivers.
- Conduct performance evaluations and address concerns or disciplinary actions as needed.
- Serve as a resource for caregivers, offering support and fostering a positive work environment.
6. Client Relationship Management
- Act as the primary point of contact for clients, addressing their care needs, concerns, and preferences.
- Conduct initial assessments and regular follow-ups to ensure client satisfaction.
- Collaborate with clients and families to update care plans and address changing needs.
7. Community Relations Management
- Build and maintain relationships with referral partners, including hospitals, physician offices, and senior living communities.
- Develop and execute outreach strategies to increase referrals and community engagement.
8. Recruitment and Staffing
- Monitor staffing needs and collaborate with HR to ensure adequate caregiver recruitment.
- Assist with caregiver onboarding and training to ensure readiness for assignments.
9. Problem-Solving and Conflict Resolution
- Address and resolve conflicts between caregivers and clients in a timely and positive manner.
- Respond promptly to incidents, complaints, and emergencies, documenting and addressing issues effectively.
- Maintain open and clear communication with caregivers and clients.
10. Compliance and Quality Assurance
- Ensure caregiver compliance with agency policies, state regulations, and client care plans.
- Conduct quality assurance checks to uphold the highest care standards.
- Stay informed about home care regulations and implement best practices in care delivery.
Qualifications:
Experience
- Senior care experience required, with 2 years of supervisory experience in home care, healthcare, or a related field preferred.
- Training or teaching experience is a plus.
Education
- High school diploma required; associate’s or bachelor’s degree in healthcare, social work, or a related field preferred.
Skills
- Exceptional communication and interpersonal skills, with the ability to build strong relationships.
- Excellent organizational and time-management abilities, capable of handling multiple priorities.
