Caring Connections for Special Needs (CCSN) improves the behavioral health and wellness of children through a family support approach. We are founded on the belief that we must focus on our children and families and follow our CORE FOCUS to improve the lives of children and families. What makes our services unique is the fact that we offer our enrolled members tools and techniques to benefit the daily life of both the child(ren) and families we serve.

CCSN is adaptable to child/family needs and looks to provide family support services such as parenting education, assistance in finding employment, and time management to meet the essential needs of the family. We strive to build trust and develop positive relationships with our members and families.

Summer Program Mentor

CCSN is hiring Mentors for our upcoming Summer Program. The Mentor position will be a temporary position that supports our members who are participating in our 2025 Summer Program. The Summer Program begins on Tuesday, May 27th and ends on Friday, August 8th.

Summer Mentors are required to attend and successfully complete training for the summer program. The training will be held on the following days:

Thursday, May 8 (2 pm to 5 pm)

Saturday, May 10 (9 am to 3 pm)

Saturday, May 17 (9 am to 3 pm)

Face to Face job shadowing (min of 4 hours during the week of Mary 19th)

Training days and times may be subject to change

Responsibilities of the Summer Mentor Position

  • Responsible for providing individual services that meet the needs of the member according to their treatment plan.
  • Manage a caseload of members that includes children and adolescents under the age of 17.
  • Responsible for completing coordination of care and parent contact in accordance with the CCSN policy.
  • Maintain compliance with company policies and procedures.
  • Complies with HIPAA guidelines.
  • Follows Duty to Report and Therapeutic Boundaries
  • Coordinates services internally and externally and in accordance with the member’s treatment plan
  • Assists children to meet their goals in accordance with their treatment plan.
  • Actively communicates with the members behavioral health home about any member concerns, issues, or achievements.
  • Provides services on an individual basis, small groups, assist with transportation to and from services and to summer program activities.
  • Provides direction and follow-up to families and individuals regarding their concerns and issues.
  • Creates a safe environment for the children and their families.
  • Actively communicates with site staff about any changes with members.
  • Timely documentation of all services provided in accordance with the company’s policies and procedures.
  • Reports all incidents in “real time “to the Supervisor and completes appropriate forms/documentations as directed by the Supervisor.
  • Attends mandatory weekly Supervision facilitated by the CCSN Clinical Manager.
  • Communicates with Site Supervisor about hours worked, anticipated overtime, scheduled time off and absences from work.
  • Adhere to assigned work schedule and communicate with your supervisor about any barriers to reporting to work as scheduled.

  • Must be able to work under pressure and meet deadlines while maintaining a positive attitude and providing exemplary customer service.
  • Ability to work independently and carry out assignments to completion with the parameters of instructions given, prescribed routines and standard accepted practices.
  • Excellent verbal communication skills, including ability to effectively communicate with all age groups of children and parents, guardians, and family members.
  • Strong written communications skills for documenting member services in the electronic health record.
  • Must have excellent initiative, good judgement, and the ability to make timely decisions.
  • Recognition of priority issues, quick response and execution of plans and processes
  • Understand and be supportive of the organization’s stated mission, purpose, and core values.

What you Need - Education and Work Experience

Requirements to be a Mentor Behavioral Health Paraprofessional

  • Have a high school diploma or high school equivalency diploma and:
  • Preferred 1 year of behavioral health work experience
  • Preferred 1 year of work experience with children and families

Other required knowledge, skills, and abilities

  • Must be 21 years of age or older, in accordance with Dept. of Health Services R9-20-204.
  • Excellent computer proficiency (MS Office- Word, Excel, and Outlook)
  • Must have a current/unexpired Driver’s license and maintain clean driver’s record for the past 39 months. A MVD will be completed during the pre-employment background.
  • Have reliable transportation to arrive to work as scheduled.
  • Provide and maintain current proof of insurance and registration.
  • Must maintain a current/valid/unrestricted AZ DPS Level 1 Fingerprint Card.
  • Have current CPR/First Aid certification.
  • TB test result within the past year. Results must be provided prior to the first day of the summer program.
  • Must be able to meet training and agency compliance requirements for the position.
  • Ability and willingness to receive coaching and feedback.

Physical Requirements

  • An individual may qualify for this position if the individual is able to "perform safely" the essential functions of the position with or without reasonable accommodations.
  • Able to perform the essential job functions safely and successfully.
  • Maintain regular, punctual attendance consistent with the ADA, FMLA and other Federal, State and Local standards.
  • Must be able to lift and carry up to 50 lbs.
  • Must be able to talk, listen and speak clearly on telephone, and face to face with parents and guardians.

Job Types: Full-time, Temporary

Pay: $15.00 - $17.00 per hour

Expected hours: 40 per week

Schedule:

  • 8 hour shift
  • Day shift
  • Monday to Friday
  • Weekends as needed

  • Children's Behavioral Health Services: 1 year (Required)
  • Behavioral Health Services: 2 years (Required)

License/Certification:

  • AZ DPS Level 1 Fingerprint Clearance Card (Preferred)
