Responsibilities of the Contract Assessor
The site assessor’s primary role is to ensure that the comprehensive needs assessments, both for new referrals and current members. The site assessor will complete assessments appropriately, timely, accurately, thoroughly.
Under the direct supervision of the CCSN Clinical Supervisor, BHP, the site assessor is responsible for completing all new member assessments and updated assessments for current members in a timely manner.
- Completes clinical assessment using the integrated ICANOTE assessment tool on all assigned cases within 24 hours of meeting with the family. The tool includes:
- Assessment (electronically signed and submitted to the Clinical Supervisor, BHP
- Completes assessments with attention to detail (clinically important data that justify medical necessity, frequency, intensity, specificity, and duration).
- Select assessment code-H0031 and input one (1) unit for the billing unit.
- Treatment plan (electronically signed and submitted to the Clinical Supervisor, BHP)
- Select the Case Management code T1016, add start and end times which will add total units to billed.
- Testing Batteries:
- CALOCUS (Printed/Signed and emailed to Clinical Supervisor, BHP for signature)
- DLA-20, if applicable will be completed within ICANOTES and submit electronically to the Clinical Supervisor, BHP
- ASQ (Printed/Signed/upload to the members ICANOTE chart)
- ROI and Consent Forms
- Ensure that the forms have been signed by the members guardians
- Transportation Administrative forms (if applicable)
- Printed/Signed/uploaded to the members ICANOTE chart
- Substance Use Screening (for ages 11-17) must be completed
- Printed/Signed/uploaded to the members ICANOTE chart
- Makes appropriate recommendations for service for members (Community Resources, Behavioral Health Homes information).
- Demonstrates the ability to write clinically and diagnose appropriately, justifying all diagnoses utilizing DSM language.
- Communicates timely with referral sources, teams, CCSN Clinical Supervisor, BHP, CCSN Site Supervisor, and CCSN administrative staff about the status of assigned cases, changes in member status, i.e. discharges, transfer of site location and scheduling of initial services to ensure compliance with the seven (7) day window.
- Resubmits to Clinical Supervisor, BHP, the requested updates/changes/corrections of work within 12 hours of receipt.
- Completes health record documentation within 24 hours of service contact.
- Submit non-billable DAP notes for attempted contacts. (Dates of notes must match the date on the assessment, ISP, consent forms, testing batteries, etc.)
- Documents status updates in the established assessment tracking document.
- Using “secure” email, send a copy of the ASSESSMENT & ISP to members health home and /or PCP.
- Print email sent to health home & Upload to member’s chart.
- Uploads pertinent documentation to members ICANOTES chart, including but not limited to medical documentation, behavioral evaluations, and court paperwork.
- Check e-mail daily and respond to CCSN Clinical Supervisor, CCSN Site Supervisor, and CCSN Administrative staff immediately.
- Provides assessment support to other CCSN locations as requested by the site supervisor and/or administrative medical records team.
- Completes the six (6) month CALOCUS and transportation request (if applicable) for members assigned by the medical records department.
- Attends initial and ongoing training for all assessors and clinical staff (monthly, quarterly, and yearly training).
- Attends weekly team meetings, quarterly meetings and all mandatory meetings that may impact their role/process.
- Demonstrates the ability to organize, prioritize required and assigned job duties and functions with minimal direct supervision.
- Complies with all State and Federal Program requirements.
- Demonstrates flexibility as it relates to a change of work hours on short notice; performs other duties as required.
- Creates a safe and secure environment for the children and their families during the assessment process.
- Uphold standards of confidentiality according to agency, state, and federal standards i.e., HIPAA, 42 CFR
- Utilize knowledge from child welfare practices and mental health insight to generate a comprehensive service plan for each member.
- Follows CCSN Mandated Reported Policy that adheres to Mandated Reporter Arizona Law, ARS 13-260.
- Follows CCSN Duty to Warn Policy that adheres to Public Health and Safety Arizona Law, ARS 36-517.02.
- Follows CCSN Code of Ethics Policy
- Provides direction and follow-up to families and individuals regarding their concerns and issues.
- Provides information and referral assistance to families.
- Assists with Case Management
- Maintains compliance with all company policies and procedures.
What you Need - Education and Work Experience required for the BHT-Contract Assessor
- Meet the requirements of a BHT in accordance with the AHCCCS regulations
- Minimum of two (2) years of behavioral health work experience, working with children and families.
- Minimum of one (1) year of experience as a behavioral health assessor working in a provider organization.
- Post secondary education is preferred.
Location of Contract Assessor
CCSN has locations in Benson, Tucson, Sierra Vista, Douglas and Safford. The contract assessor can be located in any one of the CCSN locations.
