We are looking for an experienced Care Manager to join our team. The successful candidate will be responsible for providing comprehensive care management services to clients, including assessment, planning, coordination, monitoring, and advocacy. The Care Manager will work closely with clients and their families to develop and implement individualized care plans that address the client's needs and goals. The ideal candidate will have a strong background in caregiving, social work, or a related field and a passion for providing quality care.
Responsibilities:
- Provide support to clients and their families.
- Assess clients' needs and develop care plans to meet those needs.
- Monitor client progress and adjust care plans as needed.
- Develop and maintain relationships with community partners and other professionals to ensure clients receive the best possible care.
- Provide education and resources to clients and their families about available services.
- Ensure compliance with state and federal regulations related to client care.
- Document all client interactions in a timely manner.
- Able to fluently communicate with Spanish and English speakers.
Job Location: Hybrid
- Duties will be "Hybrid," meaning day-to-day operations will be done partially from our home office and primarily in the field.
